Justin Njinmah came on in the 66th minute and tapped the winner four minutes later after Cologne goalkeeper Marvin Schwäbe missed a cross from Mitchell Weiser in the 100th meeting between the sides.
Cologne fought to avoid a first defeat in four matches and Bremen captain Marco Friedl needed to clear on the goal-line to prevent Weiser from heading into his own net off a corner kick.
It was Bremen's third straight 1-0 victory on the road, the others coming at Mainz and champions Bayern Munich. The only other time they won three games in a row away from home without conceding was in 1981-82.
Bremen rose to seventh and are 13 points clear of the danger zone with Cologne in the relegation play-off spot. Cologne failed to draw closer to Union Berlin who are five points ahead of them.
The 22nd matchday continues on the weekend with leaders Bayer Leverkusen at Heidenheim on Saturday and second-placed champions Bayern Munich at Bochum on Sunday.
