The program brings people together to meet new faces, build friendships and take part in meaningful activities that support independence and confidence.
Options include walking, yoga and gentle exercise, day trips and group outings, gardening, card games and arts and craft sessions, such as painting and jewellery making.
Activities can also be shaped around the interests and ideas of participants.
“Building social connections is an important part of supporting people to remain active, engaged and connected to their community,” Kyabram District Health Service social support innovation co-ordinator Nikita Saunders said.
“KyConnex provides a safe and supportive space where people can participate in activities they enjoy, meet others and feel a sense of belonging.”
The program is open to people living in the Kyabram, Tongala, Stanhope and Girgarre areas.
For those aged over 65, a My Aged Care referral is required.
The KyConnex team can assist community members through the referral process and help with eligibility and funding enquiries.
You can find My Aged Care at myagedcare.gov.au
The KyConnex Social Program is subsidised through Commonwealth Home Support Program funding and Support At Home Packages.
Small out-of-pocket fees apply to activities; though indicative costs have not been provided, participants are encouraged to discuss Support At Home pricing with their care partner.
Bookings are essential for all sessions.
